Chapter 90 A small test of one’s skills
The gastroenterology department is a scene of extreme activity.
Beside the white curtains, Chief Physician Niijima, wearing a stethoscope, was diagnosing an elderly man's illness. His hands were constantly pressing on the abdomen, and his expression was extremely serious. The stethoscope was sometimes placed below the abdomen to listen to bowel sounds.
He kept asking questions.
Beside her, Dr. Horie was typing on a keyboard, recording the patient's specific symptoms and compiling them into a medical record.
Dr. Toda also joined in. He had studied under Chief Physician Niijima for many years and was fully capable of handling things on his own. The two voices rose and fell in the consultation room, while another doctor, Dr. Kikuchi, was responsible for taking notes.
In the corridor, several members of the Business Law Society sat in chairs chatting and waiting.
Because of these people's status, everyone in the department speaks softly and tries their best to be the best in terms of attitude.
"Please describe the specific type of pain you're experiencing. Is it tenderness or a rebound pain?" Director Niijima asked the elderly man carefully.
"I can't quite explain it, but it hurts when I press on it, though it's not particularly painful..."
Based on the old man's not-so-detailed description, Director Niijima had already formed a general judgment.
"Do you usually experience bloating, acid reflux, or a burning sensation behind your sternum after meals?" He placed the stethoscope against the area and said casually, "It's right behind your sternum, in this spot."
"I have poor digestion, and I often experience bloating and acid reflux. I'm getting old, so this is normal, right?"
"It might be gastritis. Tell this doctor what kind of medications you usually take when you come here, and I'll be able to make a diagnosis with just a glance. If there is inflammation, an endoscopy may be necessary."
"How could there be inflammation? I don't feel anything."
“It’s alright, sir. Please come over here.” Dr. Horie smiled and gently tugged at the old man’s sleeve, gesturing for him to sit down in another chair.
Then.
An electronic announcement calling out to patients came from the entrance of the outpatient clinic.
The next patient pushed open the door and stepped inside.
……
After the meeting, the interns walked out of the conference room one after another, each rubbing their sore necks, clearly relieved.
Sitting upright for two whole hours was somewhat of a torment.
There was no other way; the whole thing was being filmed by cameras, and the purpose of calling them here was to film them.
However, Lin Ze still felt that he had gained something overall.
The video documented the entire process from the patient getting off the ambulance to entering the emergency operating room, the collaborative planning of the surgical plan by doctors from various departments, the final approval of the plan by the professor of the First Surgery Department, and the successful completion of the surgery.
The interns watching were extremely nervous, their hearts pounding in their throats along with the doctor in the film.
Undoubtedly, this surgery achieved a textbook success, once again proving the top-notch level of the University of Tokyo Hospital. Chief Surgeon Minamino, who was the lead surgeon in the thoracic surgery department, stole the show.
The emergency treatment provided by the ER doctor who accompanied the ambulance, the precise assessment by the radiology department, and the multidisciplinary surgical collaboration of the surgical experts—all these materials are extremely well-done.
However, for Lin Ze and his group, the biggest feeling was that Section Chief Nanye's skills were so superb.
"That way, even if I get a good scolding, I won't feel resentful."
"After all, the person who taught us is truly talented. I want to become a doctor like Section Chief Minamino in the future. That would be so prestigious."
"Let's get our medical license first before we talk about anything else; this hurdle alone is difficult enough."
"By the way, Section Chief Minamino is also a student at our University of Tokyo's Faculty of Medicine? So he should be our senior."
"That's true, but he was a senior from over twenty years ago."
Listening to everyone's discussion.
Sano Chizuru said to the two of them, "Shouldn't we hurry back? I heard the department is very busy right now. Dr. Horie just sent a message asking when our meeting will end."
"can."
Lin Ze and his group didn't dare to delay along the way. When they reached the seventh floor of the central outpatient building, they immediately saw the patients waiting for treatment in the corridor.
They quickly went to the department, put down their notebooks, and put on their stethoscopes.
They threw themselves into their busy work of diagnosis and treatment.
Not a moment later.
In the gastroenterology department.
With the addition of the three people, the workload was noticeably lighter.
Because of his outstanding performance, Lin Ze was assigned to learn diagnostic and treatment techniques with Director Niijima, while Miyagi Suzuo and Chizuru were assigned to record medical records. This gave Dr. Horie a huge sigh of relief—she had been wanting to go to the toilet for a long time and had been holding it in.
After apologizing in the office, she quickly went to the restroom.
"Name."
"The ancient house is in good health."
"age."
"I'm fifty-eight years old."
"Please tell me about your past medical history."
Lin Ze's calm voice rang out in the department as he looked at the elderly man with half of his hair turned white.
After this person sat down.
He immediately smelled a foul odor, very faint, but definitely detectable if you sniffed carefully. This was an innate ability of Lin Ze, and one of the reasons why he had never been interested in internal medicine before.
Because of his illness, Lin Ze believed there was no way to hide it.
He doesn't need complicated diagnosis; he can roughly determine the cause of the illness just by moving his nose.
Since joining the internal medicine department, Lin Ze has seen many patients, both big and small, and has gained a clearer understanding of his abilities.
For example, the old man in front of us clearly smelled of rotting peaches, which meant that his illness was in his intestines.
"I have high blood pressure, I had pleurisy when I was younger, and I had hernia surgery a few years ago," the old man replied. "What are your usual eating habits, and do you drink alcohol or smoke—"
Before Lin Ze could finish asking his usual question...
Beside him, Dr. Niijima suddenly adjusted his glasses.
"Lin Ze."
"Hmm, what's wrong, Director?"
"You've been learning from me for over a week now, why don't you try treating me?" Director Niijima's eyes held a faint smile.
It happened suddenly.
Everyone in the department was stunned for a moment.
Interns always need to be trained. When Director Shinjima is dealing with patients, he also teaches Lin Ze and the others everything in detail.
It seems they want to test how well he's learned.
Lin Ze was also slightly taken aback.
“It’s alright, don’t feel pressured. I’ll point out any mistakes in your judgment,” Director Niijima continued.
"Okay, let me try."
However, to the surprise of the other doctors in the department, Lin Ze did not make any excuses and instead stood up on his own initiative.
He looked at the director of the new island.
Even the doctor, who was listening attentively to the patient's heart, couldn't help but turn around to watch this scene.
The busy work in the department was not enjoyable, which the doctors were naturally happy about.
"I have to go into battle after only a week of training... How scary." Sano Chizuru glanced at Miyagi Suzuo, and the two looked at each other.
Chizuru murmured to herself.
Then the two continued typing, but kept a close eye on Lin Ze's movements.
Seeing that Lin Ze was not intimidated at all, Director Niijima put down his stethoscope.
"Don't worry, I'll make room for you right away."
Upon hearing this, everyone couldn't help but laugh.
Lin Ze moved to the seat reserved for attending physicians, spreading his long legs as he sat down. He put the stethoscope on his ears and unbuttoned the shirt of the elderly man in front of him.
"Ask him yourself. Miyagi will take care of your medical records," Director Niijima said casually.
"I'll press on your abdomen. Tell me where you feel pain," Lin Ze said patiently to the old man after agreeing.
"please."
Do you usually experience abdominal pain or bloating?
"No, it happens very occasionally, but it's probably because I ate something cold."
“Abnormal bowel sounds,” Lin Ze quickly placed his stethoscope against the intestines, his calm voice echoing in the department: “Is diarrhea frequent?”
"Yes, and it happens quite often, sometimes with mucus."
Does your lower left abdomen hurt?
"It hurts a little."
"Hmm, pull down your pants and let me take a look."
"Pants?"
"Yes."
The old man was somewhat puzzled and couldn't help but glance at Director Shinjima beside him.
"Just listen to him." Director Niijima did not object.
After listening, the old man had no choice but to roll up his pants so that Lin Ze could observe his lower limbs for a moment.
“Alright, pull down your pants,” Lin Ze said, reaching out without even turning his head, “flashlight.”
The flashlights used in hospitals are extremely small and specialized, generally used for examining specific areas, and are basic diagnostic tools.
Director Shinjima, who had his hands behind his back, showed no other expression after Lin Ze spoke. He simply reached out and took the flashlight from the side and handed it to him.
This scene made the village doctor, who had just had a moment to rest, swallow hard.
Are they just going to make the director their assistant like that? This new intern is truly fearless, or perhaps it's just ignorance that breeds recklessness...
Lin Ze, of course, had no idea what others were thinking. He turned on his flashlight, shone it into the patient's eyes, and pried open the corners of his eyes.
"The preliminary diagnosis is ulcerative colitis," he concluded.
"The basis for the diagnosis," Director Niijima said calmly.
"Obvious diarrhea symptoms, dull pain in the lower left abdomen, mucus in the stool; painful nodules in the joints of the lower limbs, and uveitis in the eyes, both of which are complications."
Lin Ze said calmly.
In the department.
A moment of silence.
After he finished speaking, there was no other sound for a moment.
It wasn't until Director Niijima took off his glasses that a relieved smile appeared on his face.
"Excellent, a perfect diagnosis."
